Berkshire Family Medicine, PLLC is participating with the insurance companies listed below: 

• Aetna (except for children under 18) 
• Blue Cross/Blue Shield (most plans except ValuMed Plus and Healthy NY OptionB)
• CDPHP (most plans except Select, Medicaid, Child Health Plus, and Family Health Plus)
• Cigna (except HMO)
• HealthNow
• Medicare and Medicare Private Fee-For-Service (PFFS) plans
• Meritain Health
• MVP
• RMSCO
• Any plan that is secondary to your Medicare, except Medicaid

All co-payments must be paid at the time of service, by cash or check, as per your agreement with your insurance company.  There will be a $40 charge for checks that are returned by your bank for insufficient funds.  We do not accept credit cards.

For patients with no health insurance, payment in full is expected at the time of service, although a 40% discount will be given for such payment.

Because of the small size and independent status of this practice, Dr. Kinney cannot afford to participate with insurances that provide unusually low payment.  We regret that we are unable to accept new patients with Medicaid.   United Healthcare (including the Empire Plan) will only be accepted when it is secondary to Medicare.  We do not participate with No-Fault and Worker's Compensation.
